~ Become a part of the Zachary Brooks CHD Endowment "Saving Hearts" group by giving $1,000 or more to the Zachary Brooks CHD Endowment through the Midwest Affiliate of the American Heart Association and be recognized on the Zachary Brooks CHD Endowment Web Site with a link promoting your site. ~ Become a part of the American Heart Associations (Midwest Affiliate) Benetactor's Circle by contributing between $5,000 and $9,999 and be recognized in the American Heart Associations, Midwest Affiliate Annual Report. ~ Become part of the Life Saver Circle and designate your contributions each year to the Zachary Brooks CHD Endowment for Congenital Heart Defect Research. Life Saver Circle members receive only one solicitation per year, saving both time and money. If you are interested in joining "The Circle," for the Zachary Brooks CHD Endowment, call Laurie Loughridge at 800-677-5481 ext. 223 and tell her you want your contributions yearly to go to the Zachary Brooks CHD Endowment. ~ Become part of the "Friend" group by giving $1,000 - $2,499. ~ Beome part of the "Benetactor" Group by giving $2,500 - $4,999. ~ Become one of the "Partner" group by giving $5,000 - $9,999 ~ Become a "Champion" by giving $10,000 or above. All are recognized in the American Heart Association's, Midwest Affiliate Annual Report for being a part of the Life Circle and giving hope to the thousands born each year with a congenital heart defect (CHD). ~ Any donation of $10,000 or more will be recognized in the American Heart Association (National) Annual Report, and the American Heart Associations (Midwest Affiliate) Annual Report. A person giving $10,000 - $24,999 will be recognized as part of the Crystal Level at the American Heart Association. ~ A person giving between $25,000 and $49,999 will be part of the Bronze Level. ~ A person giving in the amount of $50,000 to $99,999 will be a part of the Silver Level. ~ Anyone giving in the amount of $100,000 to $249,999 will be added to the Gold Level |
